Telehealth Consent & Clinical Disclosure
Purpose of Telehealth
Telehealth involves the use of electronic communications to enable healthcare providers at different locations to share individual patient medical properties for the purpose of improving patient care. The services provided through SHWcare include digital health consultations, medical review, and therapy coordination.
Independent Medical Providers
By proceeding, you acknowledge that SHWcare is a technology platform and not a medical provider. All clinical evaluations and medical decisions are made exclusively by independent, US-licensed healthcare providers (the "Providers") contracted through third-party medical groups, such as MD Integrations.
Nature of the Treatment
The Providers will evaluate your medical history and health goals to determine the medical necessity of any therapy. You understand that:
- A consultation does not guarantee a prescription.
- Prescriptions are issued solely at the clinical discretion of the Provider.
- You have the right to discuss the risks and benefits of any specific therapy with your Provider during the review process.
Risks and Limitations
While telehealth offers significant convenience, there are potential risks unique to remote care:
Despite our HIPAA-compliant AES-256 encryption, there is a remote risk of security protocol failure.
A remote evaluation may lack the physical examination components of an in-person visit.
Delays in evaluation may occur due to technical interruptions.
Privacy and PHI
Your Protected Health Information (PHI) will be handled in strict accordance with our Privacy Policy. By consenting, you authorize SHWcare to share your intake data with the independent medical group and the designated specialty pharmacy for the purpose of fulfillment.
Eligibility and Geo-Restrictions
Telehealth services are only available to individuals located in states where our Providers and Pharmacy partners are licensed to operate. You agree to provide accurate information regarding your physical location at the time of service.
Medical Emergencies
If you are experiencing a medical emergency, you must dial 911 or visit the nearest emergency room immediately. SHWcare and its associated Providers do not provide emergency medical services.

- You have read and understood the information provided above.
- You are at least 18 years of age and are legally authorized to consent.
- You voluntarily choose to participate in telehealth services through the SHWcare platform.
