For Health Care Operations:

We may use your information for platform maintenance, quality assessment, and administrative purposes to ensure our services remain efficient and secure.

  • Right to Inspect and Copy: You have the right to inspect and receive a copy of your medical records and billing records.
  • Right to Amend: If you feel that the medical information we have about you is incorrect or incomplete, you may ask us to amend the information.
  • Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI.
  • Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you for treatment, payment, or health care operations.

In accordance with HIPAA Security Rules, SHWcare employs enterprise-grade administrative, physical, and technical safeguards. All data transmitted through our platform is encrypted using AES-256 standards to prevent unauthorized access or disclosure.

5.0 Changes to This Notice

We reserve the right to change this notice. We will post a copy of the current notice on our website with the effective date clearly marked.
